I feel like the only way it will be #5 is if Moffat really REALLY wants to troll the fandom. Though, I've also got a feeling that he might troll the fandom by having Eleven jump out of his timeline at the beginning of the special (into a random moment in Ten's life) and have the whole episode as a run-of-the-mill double!Doctor adventure with no mention of the mysterious Hurt, leaving fandom to stew over the resolution of that particular puzzle until Christmas or beyond... (it would be a good 'draw' for getting reluctant viewers to continue watching with 12, just to find out what's going on)
Number 3 struck me as something that hasn't been suggested before (or, at least, not suggested overwhealmingly) but could still make sense. More or less. Also, wondering now about other people spinning around in the Doctor's timeline. I think it might be quite a bit of interesting if the Doctor's timeline were standing in as the MATRIX and all that implies...
